When we speak of the ACP athletics, we speak of all those coaches and student athletes who practice, compete and above all represent Antonian with a high degree of honor and character. We are proud of each and every one of them.
Athletic Documents
ATHLETIC FORMS:
The new Athletic Physical Packet Forms have been emailed to all the parents. If you have not received one please email the webmaster, Ms. Misty Gonzales.
Football Players
All football players must have health insurance and show proof of coverage. Submit a photocopy of your child’s insurance card and sign a waiver. You have the option of purchasing the school’s insurance provided by Student Assurance Services as secondary insurance. If your child is not covered by health insurance, you must purchase the school’s insurance. The insurance application and waiver are provided below. Please attach the waiver and proof of insurance to the physical packet. NOTE: If you are purchasing the school’s insurance, please make a photocopy of the completed application and personal check as proof of insurance and attach to the physical packet.
